Question Prompt Sheet (QPS)
Many people with upper limb loss have questions or concerns that they want to discuss with their healthcare team. During busy clinic visits people may forget to ask their questions. We created a hand/arm transplant Question Prompt Sheet, which is a list of questions to help you get the information that you want about hand/arm transplantation. You can use this list to identify questions important to you and help you to remember what to ask the transplant team.
